Please review this Consent to Care, which explains what to expect from treatment via telehealth by [MG Health Tech Inc affiliated professional entity name(s)] (collectively, "we," "our," or "us").
1. Consent to Telehealth
By participating, you voluntarily consent to receive health care services from us via telehealth. You understand that these services may include care-gap review, health recommendations, remote consultations, and other treatment, and that no specific result or cure has been promised. This consent remains in effect for as long as you receive services from us, unless you revoke it in writing.
2. Your Provider's Credentials
Your provider's credentials are made available to you before your appointment. Direct any questions about them to your provider.
3. What Telehealth Is, and Its Risks and Benefits
We provide care through licensed professionals using telecommunications technology ("telehealth"). Services may include chart review, remote prescribing, scheduling, refill reminders, health-information sharing, and non-clinical support such as patient education.

Benefits may include improved access to care from your home, more efficient evaluation and management, and access to appropriate specialists.

Risks may include: delays in evaluation or treatment due to technology failures; occasional poor transmission quality requiring a rescheduled visit or an in-person visit; and, rarely, that a condition may not be appropriately assessed remotely.
We use security measures designed to protect the confidentiality and integrity of your information, though no method is perfectly secure.
4. Confidentiality and Its Limits
Your care team will protect your health information as required by law. There are limited exceptions where disclosure may occur, including: a serious and imminent threat of harm to you or others; suspected abuse or neglect of a child or a vulnerable adult; a court order or subpoena; consultation among treating providers or supervision; sharing with your health plan to obtain payment; disclosures described in our HIPAA Notice of Privacy Practices; and as otherwise permitted or required by law.
5. Treatment and Confidentiality of Minors
Consent for treatment of a minor may generally only be provided by the minor's current legal guardian. If you are consenting on behalf of a minor, you represent that you are the parent or legal guardian with authority to make health care decisions for that minor.
6. Your Acknowledgments
By clicking "I agree" (or similar), or by receiving telehealth services from us, you acknowledge and agree that:
1. You have read and understand this Consent, had a chance to ask questions, and consent to receive services via telehealth. Telehealth does not replace your relationship with your primary care provider, and your provider decides whether your needs are appropriate for a telehealth visit.
2. If consenting for a minor, you have legal authority to do so.
3. Alternatives such as in-person care are available; some steps (e.g., labs) may be performed at your location or a facility at your provider's direction.
4. Expected benefits are not guaranteed.
5. You may review your provider's credentials before the visit.
6. Telehealth may involve electronic transmission of your information to practitioners who may be located elsewhere, including out of state.
7. We will implement reasonable safeguards to protect your health information.
8. Your information may be shared for scheduling and billing, and support personnel may be present to operate the technology; you will be told of their presence and may ask them to leave, omit sensitive details, or end the visit.
9. Technical failures can occur; you agree to hold us harmless for information lost due to such failures beyond our control.
10. You may request a copy of your medical records (reasonable cost of preparation and delivery may apply).
11. You may withhold or withdraw consent to telehealth at any time without affecting your right to future care. In an emergency, your telehealth provider cannot connect you directly to local emergency services.
